Shareholders  who hold their shares in two broad categories:  Registered held by you in your name and Street Name (shares held with a bank or brokerage firm as custodian)

Registered shareholders have certificates issued by Artes Medical and appear on the registered shareholder list.

Registered shareholders should return their proxy cards directly to MacKenzie Partners either via email (proxy@mackenziepartners.com) or via fax (+1 212 929 0308).  Registered shareholders who have not received their proxy card can request a duplicate by contacting us (see below).

Registered shareholders cannot vote via telephone or internet.

Street Name shareholders can vote via telephone and/or internet.  The instructions to do so will be included on the proxy card they receive from their bank or broker.  If a Street Name shareholder does not receive a proxy card they can contact their custodian (bank or broker) and either request their custodian vote their shares on our blue card or provide the pin # associated with their account for the blue card.  Once they receive the pin # they can enter it at www.proxyvote.com and follow the instructions on that website to vote Dr. Shack’s agenda items.

It is important to disregard the White proxy card and not complete it, even as a protest vote, as this may possibly cancel their vote on our Blue proxy card.

Broadridge advises if Street Name holders speak with their custodians to request a pin # they should also confirm their mailing address is complete and accurate.
